Drawn from aviation, engineering, medicine, military operations, and energy, these cases span a century of high-stakes decision-making. The thinking failures they reveal are not random. They are patterned, predictable, and preventable.
Grounded in Richard Paul and Linda Elder's critical thinking framework, the book is organized around four categories of intellectual failure: the foundation of clear thought, the quality of evidence, the direction of attention, and the character of the thinker. A fifth chapter examines the accident at Three Mile Island — the one case in this book where the cascade stopped — as a synthesis of all 15 standards holding under extreme pressure.
A Postlude presents an unedited exchange with an AI language model on whether these standards can survive in an information ecosystem increasingly shaped by systems that generate confident answers without knowing whether those answers are true. The reader is invited to evaluate it by the same standards applied throughout the book.

Peter H. Tyson is a retired U.S. Navy Commander with 21 years of service as a helicopter pilot, developmental test pilot, and Aviation Engineering Duty Officer.
